Community Outreach Coordinator

The Community Outreach Coordinator is responsible for outreach to the uninsured, under insured and medically underserved areas and populations as identified in the Advantage Care FQHC Community Needs Study.  Community Outreach Coordinator will identify those in need of services which include, but are not limited to, preventive primary health care services, including care for chronic and communicable diseases, dental services, mental health services, pediatric care and immunizations.

  • Work with administration to develop and implement a comprehensive and innovative community outreach plan targeting individual, corporate, school, places of worship and other organizations.
  • Assist in obtaining essential support services such as Medicaid, Exchange and other managed care health insurance coverage.
  • Facilitate the ongoing community and center based patient registration with participating insurance companies.
  • Provides basic health information and education to patients.
  • Assist in obtaining transportation, translation services and referrals, as needed.
  • Research and assist in developing plans for expanding the Advantage Care presence.
  • Serve as an ambassador of the mission of Advantage Care as a Federally Qualified Patient Centered Medical Home (PCMH) in all interactions with current and potential partners.
  • Liaison with insurance companies to review panels and promote patient enrollment in health center.
  • Oversee website and social media campaign.
  • Compliance with social distancing, adherence to PPE and infection protocols
  • High School graduate/GED minimum required; Bachelor’s degree, preferred. Minimum 1 year of community outreach and marketing experience.
  • Ability to communicate in English and Spanish, both orally and written.
  • Possesses basic knowledge of target populations’ ethnic and medical needs.
  • Strong interpersonal and communication skills; the ability to demonstrate empathy and cultural sensitivity.
  • Ability to relate to patients, community organization workers and representatives.
  • Ability to work independently, works with medical and mental health professionals in a multi-disciplinary team.
  • Relevant experience case management, data management and event planning skills required.
  • Knowledge of healthcare management information applications and systems necessary.
  • Proficiency in Microsoft Office and EMR experience, preferred.
  • Must be bi-lingual (English/Spanish
