A Two-Part Series in Partnership with Facktor Health
CHCANYS is pleased to partner with subject matter experts from Facktor Health to offer two virtual learning sessions focused on Expanding Behavioral Health Impact in Chronic Disease Care. These sessions are designed to build health center knowledge and capacity to expand access to behavioral health (BH) services, improve patient engagement, strengthen BH integration into chronic disease care, and improve performance across UDS measures. All CHC staff interested in strengthening BH integration at their health center are encouraged to attend, including behavioral health, clinical, and administrative leaders.
Session 1: Foundations: BH's Role in Expanding Access to Chronic Disease Care

Theme: Where We Start — Expanding Access Using BH as a Gateway
This session establishes the foundation for ongoing discussion by exploring behavioral health as a critical driver of chronic disease outcomes. Participants will examine the connection between BH and chronic disease management, with a focus on substance use disorder (SUD) and opioids as an entry point for broader integration. The session will also introduce a readiness framework to help health centers assess where they currently stand across beginning, intermediate, and advanced BH integration tiers.

Session 2: Practical Access Strategies: Expanding Care for SUD Using Hybrid Models

Building on Session 1's foundational framework, this session shifts to practical implementation. Participants will explore concrete strategies for expanding SUD access through hybrid care models, including telehealth-based approaches for Medication-Assisted Treatment (MAT), rapid access pathways for opioid use disorder, and workflow changes that improve patient engagement and retention across the care team.
