Join CHCANYS for an exciting learning session focusing on Addressing Social Risk, Utilizing Referral Platforms and Engaging Community Partners. During this session, health center speakers will offer insights on innovative ways to address identified social risk factors and their use of referral platforms to act on social determinants of health data to support patients' health. Attendees will hear from Public Health Solutions, a community-based organization that works to build community pathways and connect New Yorkers to services that work overcome social and structural barriers to optimal patient health. Presenters will also highlight promising practices to address social needs and close gaps to improve health outcomes and achieve health equity.
