Register Today: AAPCHO's Navigating Neighborhood Context and Transportation Social Drivers of Health Learning Collaborative 

Where people live can directly influence their health. The Association of Asian Pacific Community Health Organizations (AAPCHO) will facilitate this learning collaborative for health centers to explore effective strategies to screen and address SDOH including transportation, environmental health, and neighborhood safety and sense of community. Participants will also have the opportunity to review and evaluate a closed-loop SDOH referral protocol that can be implemented at their health centers. The learning collaborative consists of four 90-minute sessions, held on Wednesdays in April and May at 9:30-11am HT / 12:30-2pm PT / 1-2:30pm CT / 3:30-5pm ET. Learning objectives 

  • Understand the neighborhood and environment SDOH that affect Asian Americans (AA), Native Hawaiians (NH), and Pacific Islanders (PI); 

  • Understand the unique considerations of screening for SDOH among AAs and NH/PIs; 

  • Learn from the cohort's experiences screening for and addressing SDOH and identify best practices; and 

  • Develop tools and strategies to implement closed-loop protocols at their organizations.
