This interactive map from the New York State Department of Health’s Hunger Prevention and Nutrition Assistance Program (HPNAP) helps community health centers quickly identify their designated regional food bank that distributes food to local county Emergency Food Relief Organizations (EFRO). Community Health Centers can use the tool to connect patients with food assistance resources by locating the appropriate food bank by region and accessing service-area information and contact details for seamless referrals.
