MOU for Communication and Collaboration

CHCANYS wants to ensure that we can communicate and collaborate with NY FQHCs before, during, and after emergencies or disasters. To do that, we need all NY FQHCs to complete a formal Memorandum of Understanding (MOU) that describes anticipated communication protocols using our EM communications platform, VEOCI. Submit yours today!

  • Unsure if your health center's MOU was submitted? Check our statewide status on MOU submissions click HERE.
  • Haven't submitted your health center's MOU yet? Download the MOU template HERE.
  • Complete and submit to emteam@chcanys.org ASAP, but no later than October 21, 2022.
  • When we receive your completed MOU, your designated contacts will receive log-in credentials for the VEOCI system
