Lower the Pressure Initiative

The American Heart Association has received a two-year grant to help improve blood pressure rates in New York State, thanks to the Mother Cabrini Health Foundation. The Lower the Pressure initiative will allow the American Heart Association team to enhance their existing clinical network engaged in Target: BP while expanding to new health care organizations.

The American Heart Association is also in the process of hiring a clinical practice facilitator to support organizations with assessing needs and implementing changes as this exciting program gets under way. You are invited to complete a short practice assessment https://heart.jotform.com/220414108906044 due by Thursday, March 31st, to help better understand your current activities and readiness level regarding:

  • Hypertension-related healthcare quality improvement activities and resources,
  • Blood Pressure Self-Monitoring (SMBP) or blood pressure loaner programs,
  • Other identified areas of need to help inform implementation.

All completed Practice Assessments will be reviewed, and you may be contacted for additional information as cohort/site selections are made.

Completion of this Practice Assessment does not guarantee selection into the practice cohort, equipment, or financial support from the American Heart Association.
