Join the CHCANYS Diabetes & Cardiovascular Disease Prevention and Control Partnership!

Join the upcoming DCPC Cohort launching in July 2021! Sites will participate in either the Diabetes or Cardiovascular Disease category of this year-long partnership and focus on specific strategies to improve the prevention and management of patients with pre-diabetes and diabetes or hypertension and high cholesterol.
