The Diabetes Toolkit contains:

Module 1: Guideline-Concordant Care for Adults with Type 2 Diabetes
Module 2: Diabetic Retinopathy Screening
Module 3: Chronic Kidney Disease (CKD) Early Detection

All Diabetes Toolkit Modules were developed in collaboration with New York State Department of Health’s Diabetes Program and are intended to support community health centers in offering holistic diabetes care tailored to the unique needs of their patient populations.
