New Access Point (NAP), Service Area Competition (SAC) and Look-Alike (LAL) Resource Center
CHCANYS has compiled the following tools and resources to support FQHCs as they prepare their NAP, SAC and LAL applications:
Data Support: CHCANYS can provide assistance with Form 4 (Community Characteristics). A data tool can be constructed for your health center that will help you complete this form in-house. In addition, telephone support will be available from CHCANYS data support staff. Click here to submit a request for assistance. If you plan to request assistance with Form 4, please allow one week for CHCANYS to respond.
Health Care Environmental Scan: While applicants must address factors specific to their particular communities, CHCANYS can provide an overview of the State environment. Our goal is to ensure that applicants have the most accurate and up-to-date information on the New York State health care environment and any significant statewide changes that have affected health centers’ ability to provide services. Letters of Support: If you would like to request a letter of support from CHCANYS All requests are subject to review by CHCANYS. CHCANYS can also support identifying existing 330 grantees that are contiguous to your service area. Please contact Adriana Chacón-Zguro.
Emergency Preparedness: Need assistance completing HRSA Form 10, Annual Emergency Preparedness Report? CHCANYS can help. HRSA is interested in knowing the extent to which your emergency plan is integrated with local/state/regional plans.
