New Access Point (NAP), Service Area Competition (SAC) and Look-Alike (LAL) Resource Center

CHCANYS has compiled the following tools and resources to support FQHCs as they prepare their NAP, SAC and LAL applications.

New Access Point (NAP) Resources

For New Access Point (NAP) applications, a CHCANYS letter of support is not required.  Upon request, we can provide the resources below to support your application.

  • Letters of Support: As required, CHCANYS can facilitate obtaining letters of support from New York State Department of Health and New York City Department of Health and Mental Hygiene.  All requests are subject to review by CHCANYS. If you would like to request a letter of support from New York State or New York City please complete this form.
     
  • Data SupportCHCANYS can help with Form 4 (Community Characteristics).  A data tool is available that will help you to complete this in-house. In addition, CHCANYS can provide local data on social and demographic characteristics, lists of local support services, that can be mapped for project narratives. CHCANYS data support staff can assist you with using these resources.
    • To request this assistance, please check off Data Support (Form 4) in the request form. In submitting this request, you are agreeing to these Terms of Participation.  If you plan to request assistance with Form 4, please allow one week for CHCANYS to respond.
       
  • Health Care Environmental Scan: While applicants must address factors specific to their particular communities, CHCANYS can provide an overview of the State environment. Our goal is to ensure that applicants have the most accurate and up-to-date information on the New York State health care environment and any significant statewide changes that have affected health centers’ ability to provide services.  Please complete this form.
     
  • Emergency Preparedness: CHCANYS can provide assistance completing HRSA Form 10, Annual Emergency Preparedness Report. CHCANYS has developed a number of templates and resources to help you describe the extent to which your emergency plan is integrated with local/state/regional, available here. For Service Area Competition (SAC) application, CHCANYS can provide the resources below to support your application upon request.
