Join CHCANYS on Monday September 22, 2025, at 12:00pm for the kickoff session in our dynamic hypertension learning series. These trainings are designed to position community health centers (CHC) at the forefront of advancing care delivery and achieving improved hypertension outcomes. Our subject matter experts include Dr. Andrew Moran, a program director, evaluator, and a primary care physician, and Amelia Fox, a Senior Clinical Improvement Specialist at Azara Healthcare.  


During this 90-minute engaging session, subject matter experts will:  


  • Review the recently published American College of Cardiology (ACC)/American Heart Association (AHA) hypertension guidelines and evaluate their applications for clinical practice  


  • Examine the population-level impact and cost-effectiveness of implementing the 2017 national hypertension guidelines  


  • Emphasize the role of data-driven approaches in advancing hypertension control initiatives  


  • Demonstrate the capabilities of Azara Healthcare’s AMA MAP BP functionalities in optimizing hypertension management  


Attendees will gain insights into the implementation of evidence-based strategies, and how optimization of systems’ functionalities can support better identification, treatment, and control of hypertension across patient populations.  


This session is designed for clinical leaders, care teams, and quality improvement staff looking to leverage data and evidence-based strategies to close care gaps and improve hypertension outcomes.
