Optimizing Hypertension Management with Virtual Care
The successful activation of patients with high blood pressure is key to addressing the widespread epidemic of hypertension (with a prevalence of over 40% in some New York State counties). Known as the “silent killer”, uncontrolled high blood pressure leads to numerous complications including, but not limited to, heart attack, stroke, aneurysm, heart failure, renal failure, and dementia.
One proven set of interventions to engage patients in their care is the use of various virtual care modalities, including virtual video visits, virtual exams, and remote physiological monitoring (RPM).
While a few health centers in New York State have already launched successful RPM programs, many are looking for actionable information on how to start an RPM program for hypertension management or how to improve the performance of an existing program.
To help you achieve these objectives, CHCANYS has partnered with a team of experienced developers and implementers of virtual care programs from Ingenium Digital Health Advisors to bring you a 5-part interactive webinar series packed with actionable information.
