Session 1 focuses on using Medicare care management services to strengthen and sustain teams, including billing requirements, documentation standards, and workflow design.
Session 2 focuses on integrating Community Health Workers into care management models, with attention to New York Medicaid considerations, team roles, and financial sustainability.
