Join CHCANYS on Friday, December 12, 2025, at 9:00am for another powerful session in our dynamic hypertension learning series. These trainings are designed to equip community health centers (CHCs) with data-driven tools, evidence-based strategies, and quality improvement (QI) methodologies that enhance care delivery and drive measurable improvements in hypertension outcomes.  


During this session, Dr Andrew Moran along with other subject matter expert(s) will explore how team-based care approaches and evidence-based strategies can be implemented to strengthen hypertension identification, treatment, and control across patient populations. The session will highlight how optimizing care team roles, workflows, and Self-Monitored Blood Pressure (SMBP) can enhance coordination and improve outcomes.
