CHCANYS' Nursing Sub-Committee is proud to present Nurse-led Strategies for Hypertension Control and Better Outcomes. Nurse leaders are at the forefront of advancing hypertension control through patient-centered care, education, and team-based strategies. This panel discussion will feature nurse leaders who will share insights on the critical role nursing plays in improving blood pressure control and outcomes. 

Participants will learn practical approaches, strategies, and best practices that can be applied within their own organizations to better leverage the nursing workforce and drive progress in hypertension control.  

Learning Objectives:

By the end of this session, participants will be able to:  

  • Describe the unique role of nurses in hypertension control and overall care delivery.  

  • Identify strategies and best practices led by nurse leaders that improve blood pressure management.  

  • Explore ways to optimize the nursing workforce in practice settings to strengthen hypertension outcomes.  

  • Apply practical approaches within their organizations to “move the needle” on hypertension control.
