CHCANYS hosted a four-part VBC Learning Series to help community health centers prepare for and strengthen their participation in value-based care arrangements. The series featured legal experts, health center leaders, and technical assistance providers offering actionable strategies and real-world examples.

  • Session 1: Negotiation Strategies for Managed Care Contracts and Successful Value-Based Care Arrangements
    Presented by Adam Falcone, Partner, Feldesman Leiffer LLP
    This session offered legal and strategic guidance on how to negotiate managed care contracts that include VBC terms. Topics included pay-for-performance, shared savings/risk models, federal and state protections for FQHCs, and best practices for contract language.

  • Session 2: Navigating Value-Based Care in Rural Health Centers 
    Presented by Nancy Deavers (ConnextCare) and Dr. Briannon O’Connor (Forward Leading IPA)
    This session highlighted two rural approaches to VBC. ConnextCare shared how it built care management systems, addressed health disparities, and leveraged partnerships in Oswego County. FLIPA discussed how rural FQHCs can collaborate through an IPA structure to build negotiating power, share infrastructure, and expand access to VBC opportunities.

  • Session 3: CHCANYS VBC Readiness Assessment – Insights and Next Steps
    Presented by Craig Hostetler, Principal, Hostetler Group with Advantage Care Health Centers, Finger Lakes Community Health, and Jericho Road Community Health Center
    CHCANYS shared key findings from its statewide VBC readiness assessment, covering health center capabilities across leadership, care models, data/IT, and contracting. The session also outlined how to use assessment results to prioritize investments and access CHCANYS technical assistance.

    Presented by Heidy Robertson-Cooper, HRC Healthcare Advisors
    This final session guided health centers through setting and operationalizing VBC goals. Topics included evaluating current VBC contracts, aligning internal teams around priorities, and using goal-setting frameworks to support sustainable growth in value-based care.
