We are currently seeking a Trans Health Patient Navigator to work at our 400 Broadway location.

Position Summary:  The Trans Health Patient Navigator is responsible for recruiting and working with Apicha’s trans* patients to engage and retain care. This individual will provide direct services to trans*, gender non-conforming clinic patients under the supervision of the Trans” Health Program Coordinator. They must carry out tasks that are needed to execute the medical and support service plans, including the following; developing assessment/reassessment and comprehensive care plans, accompanying patients to appointments when required, coaching patients to acquire prior authorizations, and coordinating logistics for services plan and transportation. 

Qualifications: The Trans Health Patient Navigator must have a high school diploma or GED with at least one (1) year of working experience in a related field. Having a strong socio-cultural identification or work experience with community of Color trans* individuals is required.
