Organization Overview:

The mission of the Community Health Care Association of New York State (CHCANYS) is to champion community-centered primary care in New York State through leadership, advocacy, and support of Community Health Centers. To do this, CHCANYS serves as the voice of community health centers, working to ensure that every New York State community has primary care that encompasses all aspects of each patient’s health and well-being. CHCANYS is a dynamic, team-oriented, 501(c)(3) non-profit organization, and as New York State’s Primary Care Association (PCA), advocates on behalf of and provides technical assistance to federally qualified health centers (FQHCs) throughout New York State. New York’s nearly 800 FQHCs sites provide comprehensive, community-based primary and preventative care services to 2.2 million New Yorkers annually, regardless of insurance status or ability to pay. CHCANYS is nationally recognized for its programs in health policy, performance improvement, health information technology, health system planning, and training and technical assistance (T/TA).


CHCANYS’ Clinical Excellence and Innovation (CEI) Program serves New York State’s Community Health Centers (CHC), also known as Federally Qualified Health Centers (FQHCs) as a resource hub of primary care, health equity and practice transformation, performance improvement and clinical informatics training and technical assistance. The Clinical Excellence and Innovation (CEI) Program facilitates FQHC’s success in value-based healthcare and payment arrangements by promoting health equity across vulnerable populations and disseminating evidence-based practices to maximize efficiency and performance in metrics related to health disparities, prevention, chronic disease management, and appropriate utilization; driving operational excellence by optimizing the use of clinical informatics; building competency and capacity in population health management, care management and coordination, team-based care, and patient engagement; and promoting integration with the Medical Neighborhood by building community linkages and partnerships and preparing CHCs to practice according to advanced primary care modules.

The Primary Care Transformation Assistant Director leads the strategic development and oversees the implementation of one specific CEI programmatic focus area: value-based care models. This focus area requires strong collaboration with other CEI team members that are experts in related and supporting focus areas (chronic disease management, behavioral health integration and social determinants of health) and across other CHCANYS divisions, including Policy, Reimbursement and Data & Technology. This position will be based out of CHCANYS’ New York City Office, with travel throughout New York State as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to, the following:

· Serve as a subject matter expert (SME):

o Develop expertise in assigned priority focus area, including researching published and emerging evidence-based guidance, related federal and state policy initiatives and relevant healthcare implementation science.

o Serve as an internal SME resource for CEI, all CHCANYS programs and CHCANYS clinical committee and clinical sub-committees

o Serve as a CHCANYS SME representative externally (meetings, advisory committees, partner collaborations, etc.)

o Identify new partner opportunities and develop professional relationships relevant to assigned priority area to support delivery of services within CHCANYS, CEI and health centers.

o Provide general support and technical assistance to health centers in assigned priority area related to clinical excellence, practice transformation and performance improvement, including such programs as NCQA Patient Centered Medical Home (PCMH), NYS Medicaid Value-Based Payment/Care programs, NYS’s Prevention Agenda, and FQHC Accreditation and compliance support

· Oversee the development, implementation, and growth of assigned priority area training & technical assistance (T/TA) program:

o Develop an integrated workplan, inclusive of all relevant CEI projects, to ensure a cohesive, comprehensive T/TA program

o Identify and implement opportunities to align and integrate work across CEI projects and across CHCANYS, and support related cross-team collaboration to meet CHCANYS’ strategic plan, grant deliverables and deadlines and achieve target outcomes for maximum efficiency and effectiveness

o Identify gaps and emerging opportunities for T/TA in assigned priority area

o Support CHCANYS Clinical Committee and Clinical Subcommittee involvement in advancing knowledge, disseminating information and informing CHCANYS T/TA program in assigned priority area

o Support the development of learning sessions at CHCANYS Annual Conference and other T/TA events to advance assigned priority area

o Participate in identifying funding opportunities and the development of funding proposals to advance projects and health center support in assigned priority area

· Lead select assigned statewide performance improvement (PI) projects in assigned priority area, including all aspects of the planning, coordination, execution and evaluation of activities relevant to the project(s),such as:

o Recruitment of participant health centers and community partners

o Preparation of training and educational materials

o Serve as the primary contact and relationship/administrative project manager for participating health centers and external collaborative partners

o Facilitation of on-site and remote coaching sessions, trainings, webinars, collaborative calls, etc. to support PI projects

§ Conduct health center assessments to identify areas of strength and areas in need of development around data quality, clinical performance measures and outcomes, work flows and organizational PI capacity

§ Work intensively with participant health centers to foster an organizational systems approach to PI; develop PI and health information technology capacity to promote data quality and engage in data driven change; promote practice transformation through implementation of best practices and practice guidelines, team-based care, care management and care coordination, patient self-management support, etc.

§ Aggregate and analyze project level qualitative and quantitative data for reporting purposes to participating health centers, funders and CHCANYS staff.

§ Identify and test refinement ideas for effectiveness in delivering TA

§ Participation in evaluation of project impact and refinement

o Facilitate and lead practice transformation activities (i.e. T/TA., committee meetings, and small group peer collaboratives) to encourage mutually beneficial relationships and communication among health centers

o Test new content or T/TA models, including all aspects of the planning, coordination, execution, and evaluation of activities relevant to the project(s)

· Support health center’s use of technology-enabled PI, by serving as a staff liaison with the CHCANYS Data & Technology team Program Manager and Azara Healthcare staff (the software vendor for CHCANYS’ statewide population health management platform)

· Lead program management functions in assigned priority area:

o Monitor performance for assigned projects

§ Ensure all workplan deliverables are of high quality and completed

§ Track activities and outcomes to assess progress towards goals and adherence to integrated work plan with attention to key milestones and calendar deadlines

§ Monitor and ensure budgets and expenses for assigned projects:

· Establish and monitor a process for invoices and tracking of project expenses to assess budget to actuals

· Proactively identify and mitigate financial risks to budgets and communicate problems and proposed solutions to Director and Senior Director

· Manage scope of work (SOW) and contracts with outside vendors that outline deliverable expectations and expenses that appropriate within budgets and monitor invoicing in a timely and accurate manner

o Aggregate and analyze program and project- level qualitative and quantitative data for reporting purposes to participating health centers, funders and CHCANYS staff

o Preparation of project reports relevant to assigned program area

o Provide status reports on program, projects and work activities to CEI management

o Proactively identify risks and problem solve through to resolution, escalating to supervisors as needed

o Evaluate overall programmatic impact and identify opportunities to improve and advance T/TA in assigned priority area

· Supervise and manage assigned CEI staff (language for both Program Manager and/or Coordinator):

o Provide guidance and oversight in project requirements/deliverables, including the planning, coordination, facilitation and evaluation of project activities

o Development and refinement of detailed project outlines/plans

o Provide assistance in the preparation of grant requirement status reports

o Performance management and professional development

o Performance assessments and reviews

· Partner with CEI leadership:

o Identify funding opportunities and the development of funding proposals to advance CEI programs and projects

o Develop strategy for the CEI program, such as health center needs related to transformation and PI, other content priorities, emerging topics, advancement of T/TA methods, committee structures/focus etc.

o Contribute to the development of expert knowledge on PI frameworksstrategies on leading change and priority emerging clinical and public health topics relevant to FQHC’s and clinical practice transformation.

o Assess ongoing learning needs of CEI staff and participating in the implementation of new educational and professional development content for staff

o Engage in cross team sharing of best practices, lessons learned, tools, etc.

o Support the planning, coordination, and execution of activities and meetings related to CHCANYS' clinical, educational, public health and other related events (such as CHCANYS’ Annual Conference)

· Conduct other duties as assigned to fulfill CHCANYS’s mission.


· Master’s degree (MPH, MPA, MSN, MSW, MA, etc.) in health sciences or related field

· At least three years relevant experience in Clinical Quality/Performance Improvement and project management, including program implementation, management and evaluation (familiarity with value-based care models a plus)

· Staff supervisory and management experience preferred; staff coaching, training and/or project guidance and oversight experience required

· Knowledge of and interest in community health and primary care practice environments. Experience in or with a FQHC a plus

· Solid relationship building and interpersonal skills:

· Must be able to travel approximately 25% of the time throughout NYS and within the 5 boroughs of NYC; valid driver’s license required.
