COORDINATOR, BEHAVORIAL HEALTH INTEGRATED SERVICES - SPECIALITY SERVICES

The BH Integrated Services Coordinator is a management role that includes direct patient care. Tasks include working with other managers to strengthen Callen-Lorde's DEI initiatives, managing patient care including waitlists, admissions, transfers and discharges, providing formal onboarding procedures for new staff, and providing direct clinical and administrative supervision to behavioral health providers. Within the primary care clinics, the BH Integrated Services Coordinator will work to review workflows and ensure a smooth transition for medical patients seeking behavioral health services. The BH Integrated Services Coordinator will play a significant role in assisting in the coordination of chart reviews across clinics and will work closely with HIT/IT teams to improve systems functioning for behavioral health providers. The position requires on-call duties as the first-line backup to providers. The BH Integrated Services Coordinator will complete employee evaluations and work through personnel/HR issues as assigned. The BH Integrated Services Coordinator is required to work at multiple sites, work remotely, and attend operations meetings including general operations, and quality improvement, among others.

Qualifications

Education, Experience

  • Masters Degree in Social Work and NYS licensure as an LCSW (Licensed Clinical Social Worker)
  • A minimum of three (3) years of postgraduate experience in a health /behavioral health, and/or social services setting preferred
  • Previous supervisory experience preferred
  • Knowledge of lesbian, gay, bisexual, transgender, and HIV health/ behavioral health issues and concerns
  • Demonstrated ability to work with multiple evidence-based approaches to behavioral health treatment
  • Demonstrated ability to coordinate the provision of direct social services, problem-solve, and optimize service delivery systems.

This is a full-time, 5-day-per-week position including some evening hours.
