The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.

Jordan Health is now seeking a CHIEF MEDICAL OFFICER to lead the medical component of Jordan Health focusing on population health and integration of all health care services in a community health center. Our CMO positions expansion of Jordan Health’s medical care programs and ensures the sustainability of exceptional medical services integrated within Jordan Health’s primary care centers.

The CHIEF MEDICAL OFFICER will ensure that:                                                                                                                              

  • Targets for clinical capacity and utilization are established and met and the financial success of the medical primary care program is achieved
  • Clinical outcomes are improved through evidence-based strategies and innovative care approaches
  • Organizational systems and processes support Jordan Health’s strategic direction and operational expectations are achieved
  • The CMO is regarded as an example of leadership excellence, practicing primary care medicine _% of schedule, and inspiring commitment in others

Required Qualifications:

  • Medical Degree with Board certification in a primary care medical specialty
  • 5-7 years’ clinical experience to include demonstrated people management experience
  • Unrestricted license to practice medicine in NYS, CPR certification, and DEA registration

About Jordan Health:

Jordan Health is an independent FQHC, with Level III Patient-Centered Medical Home (PCMH) designation through the National Committee on Quality Assurance. Jordan Health receives funding from the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services. Jordan Health is a network of outpatient primary care offices with providers who follow a panel of patients. While independent, Jordan Health actively collaborates with the major hospital and healthcare systems in our operating area to provide a total safety net of healthcare services.

COMPLIANCE: This position requires compliance with Jordan Health’s Compliance Program, compliance Code of Conduct, and its written policies, procedures and protocols (collectively, the “Written Standards”).  Such compliance will be an essential element considered as part of regular performance evaluation of the Compliance Database Librarian.  Failure to comply with the Written Standards (which includes the failure to report any conduct or event that potentially violates legal or compliance requirements of the Written Standards) will be met by the enforcement of disciplinary action, up to and including possible termination of employment, in accordance with Jordan Health’s Compliance Program Policy and Procedure – Addressing Instances of Non-Compliance Through Appropriate Disciplinary Actions.
