Full-Time Care Management Specialist

Responsibilities:

  • Connects with and guides families during and between well child visits as part of the primary care team in carrying out the Healthy Steps Program
  • Helps families to develop positive interactions with their children, to learn good parenting skills, and provides strategies and resources to support early childhood learning
  • Conducts developmental screening, assessment and counseling for families/caregivers who have identified concerns
  • Helps patients/families to develop treatment goals and plan of care
  • Assists care team in providing pre and post visit medical follow up
  • Provides self-management support for the patients/families
  • Assists in assessment for patient/family barriers towards care goals
  • Assists patients/families to overcome identified barriers
  • Collaborates with social work staff to provide assistance and support to patients/families with financial, home health or other social needs
  • Connects patients with behavioral health staff to provide assistance and support to patients/families for behavioral modification and/or other mental health care needs
  • Coordinates patient care services between patients/families with internal and external providers and other outside organizations
  • Connects patients/families to community resources
  • Participates in collaborative care team meetings
  • Participates in quality improvement projects
  • Other duties as assigned

Minimum Qualifications:

  • Bachelor’s degree in early childhood development, health education, social work or related human services field
  • Knowledge of child development from birth to age 3
  • Bilingual in English and Chinese (Mandarin and Cantonese) required
  • Capacity for working on weekends required
  • Strong interpersonal, writing and analytical skills
  • Experience in health care setting
  • Experience with Excel, Word, and PowerPoint
  • Motivated individual able to work in a team and be self-directed
