Understanding Health Care Disparities Using REaL Data

Launching in April 2022 and ends in December 2022! Join CHCANYS in a pilot focused on enhancing health center capacity to collect granular data on Race, Ethnicity, and Language [REaL] patient characteristics.

CHCANYS invites you to join a pilot focused on enhancing health centers' capacity to collect granular data on patients' Race, Ethnicity, and Language (REaL). The purpose of this project is to build health centers' capacity to collect and report on granular information on race and ethnicity. Health centers will work on re-designing current workflows to better capture data and explore strategies for making its data actionable to inform and address health disparities. CHCANYS will support up to 5 health centers with individualized training and technical assistance (T/TA), to best leverage REaL data to advance service integration, population health activities and chronic disease management. Participating health centers will also learn how a granular approach to REaL data collection is helpful for interpreting community health needs and practicing cultural humility in medical care, while guiding workflow re-design and targeted interventions.
