SUNY Stony Brook LCSW Pilot Pipeline Program - FQHC Site Description Request

SUNY Stony Brook and CHCANYS are requesting site descriptions to share with students interested in serving at a health center site in NYS as part of their LCSW education. Interested sites who submit a detailed description will be contacted with further information about participating in the pilot program. Sites can review their FQHC HPSA scores at the CHCANYS FQHC Map. 

If interested in participating in the pilot, please complete the site description form by Friday, October 22nd, 2021.
