Secure a spot in the NYS-HCCN Community for 2022-2025

As we look to a new year and a new iteration of the HCCN program, the NYS-HCCN will be focused on meeting the needs of our members through training and education around HIT optimization, data protection, utilization of patient-facing platforms, advanced data strategies, and other supports that will help care teams improve the health of their communities and thrive in a value-based care environment. Participating health centers can also expect the addition of a Learning Management System (LMS) that will help streamline access to NYS-HCCN resources and foster a digital community. Health centers interested in securing a seat in the NYS-HCCN community must sign a Memorandum of Agreement outlining NYS-HCCN and health center responsibilities no later than January 1st. Participation in the NYS-HCCN is free and the terms of participation are manageable.
