Register for the NYS-HCCN for a Small Group Learning Collaborative on HIE Considerations for REaL Data Collection

Later this month, the NYS-HCCN will partner with HealtheConnections and the Bronx RHIO to host a small group learning collaborative on the topic of HIE considerations for Race, Ethnicity and Language (REaL) Data Collection. Collection of REaL data is critical for stratifying populations and measures to identify health disparities and gaps in clinical care.By participating in this practice facilitation, health centers can increase the volume and improve the completeness of this data for their patient population and in the statewide health information exchange (HIE). 

The objectives of this small group learning collaborative are to: 

  • Build health center awareness of the flow of REaL data from EHR collection to the HIE 
  • Grow EHR capacity to collect REaL data 
  • Explore decision-making scenarios for data collected or received
  • Identify and discuss technology related challenges and best practices for collection of REaL data 

Sessions dates (March-May) include:  Wednesdays 3/30 , 4/20, and 5/11 - 3:00- 4:00 pm!

Note: Health center participants of Bronx RHIO and/or HealtheConnections are eligible to participate in this learning event.  Health centers interested in participating in this small group learning collaborative can submit one registration form. This collaborative will be accepting participants from 4-6 health centers on a first come first serve basis.
