Join CHCANYS for a Practice Facilitation Series on Enhancing Social Determinants of Health Workflows and Responding to patients’ social needs data. This five-part Health Center Controlled Network collaborative will meet every three weeks beginning March 30th, 2022, with 8-10 health centers. This collaborative will explore strategies to advance SDOH priorities and highlight best practices, which will improve health equity and patients' health outcomes.
The objectives for this small group learning collaborative include:
- Exploring strategies to create or optimize SDOH workflows
- Identifying and discussing staff training techniques to better engage patients when collecting SDOH data
- Operationalizing an SDOH workflow within a target population
- Identifying opportunities to build or improve SDOH data strategy (ex. enhancing your understanding of the Azara SDOH module)
- Highlighting health centers’ promising practices
