New York announced initial awards of $76 million to improve regional healthcare networks and deliver technology-enhanced primary care for New Yorkers through the federal Rural Health Transformation Program (RHTP).
Congratulations to the CHCs that received awards as lead grantees: The Chautauqua Center; East Hill Medical Center; Hudson Headwaters Health Network; The Institute for Family Health; ODA Primary Health Care Network; Sun River Health, and Refuah Health Center, and congratulations to the additional health centers that received awards in partnership with other lead applicants.
