FQHC Capabilities – Data Collection via Veoci

CHCANYS is currently building a database of New York health centers’ response-related capabilities with a brief aggregate summary of significant gaps and support needed by FQHCs. We are asking each unique FQHC network to submit their capability information to CHCANYS by early May. How to participate:

  • Please review the data points first (access PDF form preview here)
  • Appoint your organizational Point of Contact (POC) who will submit the information for your FQHC network.
