CareMessage 2024 Impact Fund: Partial Subsidies for CareMessage Adoption

CareMessage is the leading patient activation and messaging platform for safety-net healthcare organizations, helping to enhance patient communication, and improve care coordination and operational workflows. As a preferred partner of CHCANYS, CareMessage currently has up to $2M in CareMessage Impact Fund subsidies available for health center members. Established to help fulfill CareMessage’s mission of leveraging technology to improve health equity and fulfill the essential needs of underserved populations, the Impact Fund can subsidize a health center’s new adoption of the CareMessage platform to address outreach to attributed patients, reduce no-show appointments, and ultimately improve health outcomes. Additionally, CareMessage customers using the Center for Primary Care Informatics (CPCI) can implement the Azara Patient Outreach (APO) module within the DRVS platform at no additional cost. By pursuing grant funding to partially subsidize the cost of CareMessage adoption for customers, CareMessage acts as a conduit to bring available funding to the organizations best positioned to improve health equity.
