New! CHCANYS Offers New Access Point Supports We are entering the homestretch to the April 3 due date for New Access Point applications! As you make your final sprint toward the finish, we want to remind you that CHCANYS’ Health Center Support team is with you every step of the way. If you would like to access any of these supports, please contact Rebecca Gaige at rgaige@chcanys.org.
Need and Demographic Data Support: CHCANYS has produced a package of tools to assist with Form 4, Community Characteristics, and Form 9, Need for Assistance Worksheet.
Health Care Environmental Scan: While applicants must address factors specific to their particular communities, CHCANYS can provide an overview of the State environment. Our goal is to ensure that applicants have the most accurate and up-to-date information on the New York State health care environment and any significant statewide changes that have affected health centers’ ability to provide services.
PCA Letter of Support: If you would like to request a letter of support from CHCANYS, please read our updated Policy Regarding Requests for Letters of Support and complete CHCANYS’ Letter of Support Request Form. Please note, completion of the Letter of Support Request Form indicates your agreement to comply with the Policy Regarding Requests for Letters of Support. CHCANYS’ receipt of the form and accompanying documentation does not guarantee that a letter will be provided; all requests are subject to review by CHCANYS. CHCANYS can also support identifying existing 330 grantees that are contiguous to your service area.
Emergency Preparedness: Need assistance completing HRSA Form 10, Annual Emergency Preparedness Report? CHCANYS can provide assistance. HRSA is interested in knowing the extent to which your emergency plan is integrated with local/state/regional plans. To help you communicate this in your application narrative, CHCANYS is drafting language that briefly summarizes current links to the New York State Department of Health and the New York City Department of Health and Mental Hygiene in an emergency event.
We hope you will take advantage of these opportunities and resources.
