The Quality Improvement (QI) Program serves New York State’s Federally Qualified Health Centers (FQHCs) as a resource hub of quality improvement training and technical assistance. The QI Program will cultivate and support FQHC adoption of innovative clinical, dental, and behavioral health best practices. This will lead to the transformation of sustainable delivery systems as health care reform is implemented with the goal of improving health outcomes for patient populations throughout the state.
Quality Improvement Vision & Goals
In collaboration with the Clinical Committee and guided by the national medical/health home model of primary care and meaningful use, the QI Program is designed to enhance clinical, dental, behavioral health and substance abuse management by:
Serving as the lead expert in assuring the delivery of high quality collaborative projects that effectively address health disparities
Promoting best practice service delivery through cutting edge clinical initiatives
Supporting workforce development through the provision of ongoing quality improvement forums and clinical education
Assisting Health Information Technology (HIT) utilization through the CHCANYS Center for Primary Care Informatics (CPCI)
Formulating QI activities based on CPCI clinical data to assist FQHCs in achieving the Triple Aim of better care, better outcomes, and lower costs
Providing technical assistance to support FQHC practice transformation beyond medical/ health home recognition
Maintaining the CHCANYS Clinical eLibrary to assist FQHCs in strengthening core services, enhancing current programs, and building new and innovative programs
CHCANYS Clinical Committee serves as a state-wide forum for New York’s health center clinical leadership. Members of the clinical committee discuss clinical matters through a CHCANYS Clinical Committee Call that takes place every 3rd Friday of each month from 9am–10am EST (unless otherwise specified). Membership is open to all clinical leaders of health centers in New York State.
