The Community Health Care Association of New York State (CHCANYS) received the Healthy Aging Grant from the Fan Fox & Leslie R. Samuels Foundation to improve screening and treatment of clinical depression in patients over the age of 60. CHCANYS has partnered with the Institute for Family Health (Institute) to implement IMPACT – a nationally-renowned, evidence-based model in the form of a Learning Collaborative. In 18 months (May 1, 2011 through October 31, 2012) the partners will support a sustainable implementation of the IMPACT model in four New York City based federally qualified health centers (FQHCs) while capturing best practices for future replication in other locations.
The following four FQHCs, selected through an application process, will participate in PIC:
Bedford Stuyvesant Family Health Center;
Bronx Community Health Network / Comprehensive Family Care Center;
Sunset Park Family Health Center;
Morris Heights Health Center
