CHCANYS has partnered with the New York State Department of Health to create the New York State Pediatric Obesity Prevention Initiative. CHCANYS will work with Federally Qualified Health Centers (FQHCs) (including section 330 grantees and Look-Alikes) in the Metropolitan (i.e., Orange, Putnam, Rockland and Westchester counties) and Central (i.e., Cayuga, Onondaga and Oswego counties) catchment areas to address pediatric obesity prevention through a year-long learning collaborative.
Three (3) Metropolitan and four (3) Central catchment area quality improvement teams from eligible FQHCs will participates in this learning collaborative that addresses pediatric obesity. Teams will learn an organizational systems approach to implement evidence-based best practices on pediatric obesity utilizing the Care Model and the Model for Improvement. As part of the improvement process, teams will collect monthly measures that are sensitive to the changes they will be testing and implementing to track performance and results over time. Participating health center teams will receive support to initiate the adoption and spread of the Care Model so that quality improvement efforts lead to improving clinical process and health system redesign.
