New York State Cardiovascular Health Initiative CHCANYS was awarded a five year grant by the New York State Healthy Heart Program (HHP). The grant’s over arching goal is to improve cardiovascular disease rates and systems of care within New York Federally Qualified Community Health Centers (FCHCs). With this support, CHCANYS coordinates and facilitates grant initiative activities based on the core concepts of the Care Model and cardiovascular care standards (i.e., blood pressure measurement accuracy and the Joint Commission).
Over the course of the grant, participating health centers have and continue to report the following measures to allow CHCANYS to track improvement in patient cardiovascular health:
Number of Hypertensive Patients in the registry/EMR
% of Hypertensive Patients who have had 2 blood pressure readings recorded in the past 12 months
% of Hypertensive Patients who have their hypertension controlled
% of Hypertensive Patients who have a documented self management goal.
The initiative is in its third year of the project and currently focuses on improving rates of hypertension within New York State health centers.

Heart Health: Cardiovascular Guidelines for Community Health Centers
Heart Health: Cardiovascular Guidelines for Community Health Centers,a training reference manual, is the bi-product of CHCANYS' collaboration with the Healthy Heart Program of the New York State Department of Health. The goal of this manual is to provide all New York State health centers and parties interested in improving cardiovascular care within their respective institutions with information on blood pressure measurement and in-house CLIA waived point-of-care diagnostic testing guidelines. CHCANYS is pleased to offer electronic versions of the manual and a corresponding training PowerPoint presentation online through CHCANYS' website free of charge.
