HealthyHeartsNYC, a collaboration of NYU Langone Medical Center’s Department of Population Health, the Community Health Care Association of New York State (CHCANYS) and the Primary Care Information Project (PCIP) of New York City’s Department of Health and Mental Hygiene (NYC DOHMH), is a 3-year AHRQ grant-funded project to help primary care practices use the latest evidence to improve the heart health of millions of Americans. HealthyHeartsNYC is one of only seven grant recipients nationwide to serve as part of the AHRQ initiative, EvidenceNOW—Advancing Heart Health in Primary Care. EvidenceNOW supports the broad effort of the U.S. Department of Health and Human Services for Better Care, Smarter Spending, and Healthier People and is also aligned with HHS's Million Hearts® national initiative to prevent heart attacks and strokes.
To ensure that primary care practices have the support and resources they need to help their patients adopt the ABCS of cardiovascular disease prevention: Aspirin when appropriate, Blood pressure control, Cholesterol management, Smoking cessation.

Receive one year of on-site and virtual technical assistance and coaching in order to assist in the improvement of cardiovascular care by:
Optimizing your electronic health record to improve CVD care delivery
Enhancing patient self-management support through dashboards and registries
Assessing and developing practice workflows to improve billing and care coordination
Training staff on creating systems to increase efficiencies and community linkages
Obtain expert consultation from nationally recognized clinicians specializing in CVD risk reduction
Access peer-to-peer learning opportunities with your primary care colleagues
Eligible Sites must:
Be located in NYC
Be primary care focused with ≤10 FTE health care providers (MD, NP, PA)
Have implemented their EHR platform for >1 year
Have no plans to change their EHR platform in the next 18 months
Be currently connected to the Center for Primary Care Informatics (CPCI)
Not currently receive and have no plans to receive external assistance on a quality improvement initiative related to cardiovascular disease
