Literature Review on Selected DSRIP Programs: Reducing Hospital Admissions Provided in this document are brief summaries and/or descriptions of published studies on outcomes related to reducing hospital admissions for projects and strategies associated with DSRIP Domains 2, 3, and 4. The document focuses on those projects identified by New York State for planning considerations among Performing Provider Systems that are relevant to participation by FQHCs.
Reducing Hospital Readmissions: Lessons from a Multi-Hospital Initiative The Preventable Hospital Readmission Initiative was launched by the United Hospital Fund in 2011. Its first stage helped seven New York City hospitals identify the causes of preventable readmissions in their facilities; in its second stage, four of those hospitals tested and implemented specific interventions to address those causes. This report gives an overview of the initiative: how it was run, results from its two stages, and lessons learned from its various findings and interventions.
Hospital Guide to Reducing Medicaid Readmissions This guide from the Agency for Healthcare Research and Quality identifies ways evidence-based strategies to reduce readmissions can be adapted or expanded to better address the transitional care needs of the adult Medicaid population.
