FQHCs who are considering participation in collaborative DSRIP proposals will need reliable sources of data to identify and document community needs and resources.The NYS DoH DSRIP website offers suggested data resources, which may be expanded as the planning effort proceeds.
CHCANYS has identified other useful data sources and we are creating tools to help health centers access and use local data for DSRIP. Below are some publicly available resources, but health centers might also consider participation in the CHCANYS DSRIP Data Support program.
New! NYS DOH DSRIP PERFORMANCE DATA The New York State Department of Health (NYSDOH) has added detailed data on preventable hospitalizations (PQIs), chronic conditions, and readmissions among Medicaid recipients at the ZIP code and county levels in downloadable Excel-compatible formats. In addition, the much-awaited data showing the overall distribution of Medicaid enrollees by ZIP code is now available. The new data include:
1. PQIs (Preventable Hospitalizations) PQI rates by ZIP and county for ALL adult inpatient stays, not just Medicaid. The area’s observed and expected rates can be used to adjust local for differences in population age, which are known to affect the probability of PQIs.
2. Medicaid PQIs (Preventable Hospitalizations) PQI rates by ZIP and county are now available specifically for Medicaid recipients. These rates per 100,000 Medicaid enrollees can be combined into service areas. The data are available for 2011 and 2012.
4. Medicaid Readmissions A new table provides risk-adjusted data on potentially preventable readmissions among Medicaid patients, based on proprietary 3M software. The data are presented by hospital and adjusted for patient conditions, to provide a fair comparison.
5. Other Medicaid Data A detailed table provides counts of Medicaid beneficiaries in 2012 by ZIP code, broken into dual eligibles (Medicare-Medicaid) and broken into children and adults. The measures include inpatient admissions, PQIs, and ER visits.
