Natalie Levy, MD is an Associate Professor of Medicine at NYU and practices at Bellevue Hospital. Dr. Levy has a hybrid position and serves as an attending physician on the Bellevue inpatient medial wards 12 weeks a year. For the remainder of the year, Dr. Levy practices in the Bellevue outpatient medicine clinic seeing patients, precepting the NYU internal medicine residents, serving as the Director of the Primary Care Hypertension Program, and serving as the Director of the Primary Care Diabetes Program.
The Primary Care Diabetes Program consists of programs like the High A1c Referral Clinic, Diabetes Group Sessions, multiple registry based Diabetes Outreach Programs, Diabetes Recall Clinics, multiple diabetes education classes, the text based MITI program that allows for remote basal insulin titration, the newly started program where an RN calls a patient 3-7 days after a change in the diabetes regimen to confirm the change or troubleshoot barriers, and the newly started diabetes patient navigator program. Dr. Levy’s recent work studying the implementation of MITI and her current work studying the social determent of health barriers facing patients with uncontrolled insulin dependent diabetes, is supported by a Healthier World Challenge Innovation Grant from the Aetna Foundation. In 2010 Dr. Levy received the General Internal Medicine Faculty of the year award. In 2017, Dr. Levy was honored as a plenary speaker at the SGIM annual meeting, was an invited speaker about MITI at the ADA’s 77th scientific session in San Diego, and received (on behalf of MITI) the National Safety Patient Award presented to one outpatient program developed in the United States to improve patient safety.

This workshop is provided free of charge through the New York Statewide Health Center Controlled Network (NYS-HCCN).
Registration is limited to 2 individuals per health center. Space is limited, and registration will be granted on a first-come, first-serve basis.
