Consider joining our growing Trillium Health family as a Psychiatric Nurse Practitioner.
Under the direction of the Chief Medical Officer and the oversight of our staff Psychiatrist, the Psychiatric Nurse Practitioner provides comprehensive care to Trillium Health patients.
Develops an acceptable practice agreement with a collaborating physician.
Submits proof of the practice agreement with an appropriate physician, licensed to practice in New York State, to the State Education Department within 90 days of employment.
Utilizes the Scope of Practice for Psychiatric Nurse Practitioner and Standards of Practice for the Psychiatric Nurse Practitioner reflecting the scope of practice.
Maintains appropriate credentials to practice at Trillium Health and submits application as an allied health practitioner to Highland Hospital.
Primary Behavioral Health Care – evaluates and treats patients with behavioral conditions as experience and credentialing allow.
Education – Serves as a preceptor for nurse practitioner students;
Health Education and Promotion - provides teaching and information on various health topics
Team-based care- Participates actively as a member with both Trillium’s medical and behavioral health staff. Participate in weekly medication discussion rounds, and in weekly case management meetings.
Collaboration with patients, families, and all members of the multidisciplinary team (e.g. mental health counselors, nutritionist, care managers, community Health workers).
Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
Responsible for maintaining confidentiality of all patient, clients, proprietary, and protected information.
Employees are accountable for meeting performance standards of their departments. They participate in compliance audits and quality improvement plans.
Other specific duties as assigned.
Nurse Practitioner (NP) degree from an accredited school of nursing. Current licensure in
New York State as a Registered Professional Nurse and current certification in New York State as a nurse
practitioner. National certification by the American Nurses' Association (ANA) (or other certifying body)
as a nurse practitioner (adult or family, preferred). Experience working in outpatient community-based
health care settings is also preferred. Experience with individuals with behavioral health diagnoses (e.g.
depression, anxiety, bipolar, etc.) extremely helpful. An understanding of electronic medical records
(EMRs) is helpful. The ability to demonstrate teamwork, a commitment to high quality care and effective
interpersonal relations skills is imperative. Timely documentation, excellent written/oral communication
and computer skills are required. Ideal candidate has background in care coordination.
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
