We are currently recruiting for a Community Health Specialist MSM.
Responsible for providing health education interventions to diverse populations, especially those at increased risk of health disparities.
Provide community-based testing (including HIV/STD/HCV counseling and testing services) as scheduled
Work with Care Management, Clinic and other Trillium Health staff to link patients to appropriate care and/or services
Schedule, plan, and present educational and risk reduction programming to individuals and groups of clients/patients, staff, volunteers, and community members within the Finger Lakes Region
Assist with the development and implementation of new community health programs within the Finger Lakes Region
Participate in outreach and link contacts to appropriate care and/or services
Provide services, when applicable, through structured interventions for HIV-negative and HIV-positive patients/clients
Operate assigned vehicles, including outreach mobile unit
Distribute and market current and appropriate educational materials
Demonstrate developing knowledge of HIV and other health-related issues
Compile and report program and service data in electronic database(s) and/or electronic medical record, as required
Represent agency at community events
Participate and provide Trillium Health/Community Health Initiatives representation on relevant coalitions, committees, networks, etc.

Bachelor's degree preferred in Health Education, Health and Human Services or related field OR Associate's degree in Health Education, Health and Human Services or related field and 2 years of related experience.
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
