North Country Family Health Center, a Federally Qualified Health Center, is recruiting a full-time Family Practice MD or Internist to provide healthcare to both adults and children in its family practice setting. The Physician requires an M.D. or D.O.; a current, valid license to practice medicine in New York State or ability to obtain a valid license to practice medicine in New York State; strong interpersonal and communication skills with compassion, and a high level of service for our patients, parents, and staff; at least two years of experience providing medical (family healthcare) to patients and families.
The Physician will evaluate and treat the Health Center Family Practice patients. He/she will provide appropriate anticipatory guidance/education, immunizations, lab work (may identify and delegate), referrals, care planning tools, resources, etc. as appropriate. Clinical services will be provided following NCFHC Policies & Procedures and guidelines set forth by the American Academy of Family Practice, NYS Department of Health, and HRSA. Evaluation and treatment will include ordering the appropriate diagnostic tests and referrals as necessary and diagnosing and treating within the accepted standards (AAFP) of practice. Explanation of diagnosis and treatment clearly in terms that are appropriate to the parent and/or child's level of understanding, providing timely patient care for patients discharged from the hospital to reduce re-admissions, and referring patient to appropriate specialists, sub-specialists, and, community resources as necessary.
The Physician will all identify patients for whom barriers exist to effective compliance (psycho-social, education, or economical), participate in the Physician after hours telephone coverage of the Health Center patients, and participate as consulting /supervising Physician with Nurse Practitioners and/or Physician Assistants as needed. Commitment to providing healthcare to families in a medically underserved community is highly valued. Experience with an Electronic Medical Record is required.
