Metro Community Health Centers' (MCHC's) mission is to deliver high quality primary and specialty medical, dental, rehabilitative and behavioral health services to patients of all ages and abilities, including those with chronic health conditions, in culturally sensitive environments, without discrimination or regard to ability to pay. MCHC is a not-for-profit network of five (5) Federally Qualified Health Centers providing comprehensive health care services, to underserved populations, including patients with intellectual and developmental disabilities, as well as those with medically and behaviorally complex needs. This posting is for a part time Primary Care Physician who will work collaboratively with the Metro Community Health Center (MCHC) team(s) to develop treatment plans and provide primary health care services and education to patients based upon their medical and psychosocial needs. Provision of services shall be in accordance with all applicable Federal and State laws, rules and regulations and in compliance with patient’s Bill of Rights and shall be based upon evidence-based care standards.
The position's specific responsibilities will include:
Provides quality medical care to patients in accordance with generally accepted standards of professional practice and the policies of the center.
Reviews all medical histories of patients upon initial visit to the center and maintains accurate documentation within the electronic medical record system.
Coordinates medical care for patients on his/her panel
Triages walk-in patient needs and calls or referrals received at the site and coordinates medical care.
Confers with the Medical Director for clinical/administrative supervision on a regular basis.
Collaborates with the health care team to ensure proper patient care and coordination. As a member of the Patient Care Team, attends and participates in daily huddles and practice team meetings.
Assists in the completion of patient care activities related to Patient Centered Medical home and other special programs, including ongoing care planning for high risk patient groups.
Participates in ongoing comprehensive care planning and review.
Provides onsite medical interventions when necessary and appropriate.
Ensures prompt follow-up action on patients with abnormal test results or physical findings.
Refers patients to additional needed services within the MCHC Network or to an outside health care facility or health care practitioner for service(s) not available at the center.
Is at all times sensitive to cultural diversity in MCHC patient population and actively promotes sensitivity among staff on the care team.
Undertakes tasks and initiatives as assigned by the Medical Director and/or Senior Director of Operations.
Maintains current and valid licensure and certification with all insurance payers.
Fulfills all administrative duties in a timely manner, including completing and locking patient notes.
Works with the Senior Director of Operations, Health Center Managers and Nursing team members to ensure optimal scheduling and health center operations.
Holds a Medical Degree (MD) from a recognized medical school.
Currently licensed and registered as a physician with the New York State Education Department.
Board Certified or Board Eligible in a recognized medical specialty.
