Responsibilities: Provide care coordination to the most vulnerable patients of the Boriken Neighborhood Health Center. Working within a patient centered medical home model, provide medical case management services, entitlement assistance, conducts health assessments and screenings, assistance with concrete service needs, health education and wellness services, and coordinated referrals to services available through the East Harlem Council for Human Services, Inc. and other agencies. Actively participate in performance improvement and peer review process. Serve as an integral member of the multi-disciplinary care team. Perform other functions necessary for the proper care of patients and functioning of the Council.
Qualifications: Bachelor’s in Social Work or related field. Five (5) years experience providing supportive services to diverse populations preferred. Bilingual Spanish/English strongly preferred. Experience working with patients and families, including individuals with HIV/AIDS, a history of substance abuse, mental illness, domestic violence and families at risk. Dedicated to providing high quality patient-centered services.
