Licensed Clinical Social Worker (part-time position)
BROOKLYN PLAZA MEDICAL CENTER, INC., a Federally Qualified Health Center located in Fort Greene, Brooklyn, has an immediate opening for a part-time Licensed Clinical Social Worker.
Reporting to the Social Services Clinical Coordinator, the LCSW position requires initiative and the use of independent judgment to, amongst other responsibilities:
Work as part of an interdisciplinary team emphasizing a team-based approach to care for patients.
Screen and assess the patient’s current psychosocial needs and determine and implement a treatment plan of action.
Communicate relevant information to other members of the interdisciplinary treatment team verbally and via documentation on patient record.
Provides direct social work services including psychosocial assessment and individual counseling, family counseling, and group counseling including appropriate referrals to other treatment disciplines as per patient needs.
Mental Health Services: Includes mental health assessment, counseling, crisis intervention referrals to treatment.
Continuum Services: Includes care management as well as emergency, psychiatric care, community support programs inpatient and out-patient care.
Participate in in-service training programs and continuing education programs that are appropriate to job responsibilities.
Provide monthly statistics to the Social Work Department to account for time and activities performed.
Attend staff meetings and participate in QI activities and educational in-services as scheduled by the Department Head.
Assists in supervision of social services staff and social work interns.
Perform other duties as required and/or assigned.
Current LCSW licensed by the State of New York.
Experience working with underserved populations including those living with HIV/AIDS, MSM, LGBTQ, as well as those formerly incarcerated.
At least one-year of experience in a community-based health care facility from a bio-psycho-social perspective.
Computer literate and experience with electronic health records.
Excellent interpersonal, oral and written communication skills.
Ability and willingness to work on a team.
