Position Description:

The Bedford-Stuyvesant Family Health Center is currently recruiting well-qualified candidates with at least 2-4 years’ experience working in a health care environment, preferably in a health center, to fill the following position:

Family Support Worker (Part Time) 

Healthy Families—Successful Start Program

 

ROLE:                       

The Family Support Worker (FSW) is responsible for initiating and maintaining regular (at least weekly) and long-term (up to two-five years) contact/support with the families.  The FSW will also be responsible for assisting the family in establishing goals and a plan for accomplishment of those goals, as well as the assessment of the normal growth and development of the target child.  Locate and make use of appropriate community resources for participants.  Evaluate actual living conditions of participants through home visits:

 

RESPONSIBILITIES:

  1. Performs home visits as the participant’s level of intensity indicates.
  2. Ensures that home visits are scheduled in a flexible manner to accommodate participants without compromising the safety of the FSW.
  3. Assists in strengthening the parent-child relationship.
  4. Assists participants in improving their skills to optimize the home environment.
  5. Increases the family’s ability to problem-solve and assume the role of advocate for themselves and their children through utilizing strength-based techniques.
  6. Links participants to appropriate services.
  7. Addresses participants concerns and empowers them to solve their challenges.
  8. Assists with the completion of the Individual Family Support Plan (IFSP).
  9. Conducts ongoing assessments of the families’ needs and progress.
  10. Documents all contacts, attempts of contacts, visits, attempts of visits, activities, services and referrals.
  11. Documents at least 15 family strengths during each contact or visit.
  12. Accurate completion of all program forms.
  13. Gathers pertinent information from participants regarding the target child’s welfare; such as keeping doctor’s appointment, well-baby visits, immunizations, medications and nutrition.
  14. Ensures that all participant information is kept confidential as per program and federal guidelines.
  15. Participates in community events as a representative of the program as needed.
  16. Submits monthly statistical reports to Supervisor.
  17. Participates in weekly 1.5 hour supervision sessions with Supervisor. 
  18.    May plan, recommend and utilize creative outreach strategies (such as mailings, phone calls, home visits, baby showers etc.) to motivate participants to participate more fully in program services.
  19.    May conduct street outreach in order to recruit participants from local businesses, schools, hospitals, clinics, etc.
  20. May be required to work evenings and weekends for program events.      

 

EDUCATION /  EXPERIENCE/ CERTIFICATIONS

Bachelor’s Degree or Associates Degree in a human resource field preferred.  High School Diploma/GED and experience working with children & families or home visiting experience is satisfactory.

 

ABILITIES:

Ability to write assessments, service plans, progress notes, weekly reports, and general correspondence related to participants. Ability to effectively present information and respond to questions from supervisors, participants, funders, other organizations and the general public.

 

WORK SCHEDULE/ SALARY/ BENEFITS

This is a part-time position that requires some evening and weekend flexibility. Salary will depend on experience and will be accompanied by a competitive benefits package.
