Position Description:



Morris Heights Health Center (MHHC) is seeking LPN Care Manager to serve as the lead on the health and wellness of the Behavioral Health patients. The goal will be to improve the overall quality of our patients’ total wellbeing and health, including physical and emotional care. This manager will work closely with the Psychiatrist and Social Workers to assist in the delivery of comprehensive care.


  • Assist psychiatrists and their patients in the Metabolic Monitoring protocol
  • Work with the Administrative team to prepare for Joint Commission Certification and OMH re-licensing
  • Assist patients in obtaining their yearly health exams, including primary care and specialty services
  • Create user friendly trainings on topics that affect the health of Behavioral Health patients ie. smoking cessation, diabetes, weight control
  • Serve as a medical educator for patients and staff regarding common medical concerns, ie. side effects of medications, warning signs of diseases, etc
  • Run groups for the significant others of Behavioral Health patients to address topics such as what to expect in therapy, importance of medication compliance and importance of keeping appointments, etc.
  • Become a link between the Psychiatrist and the patients Primary Medical Provider, both at MHHC and outside clinics, to address any and all medical concerns that may arise
  • Be the patient advocate for the Behavioral Health department to work with patient’s concerns in a timely and professional manner
  • Administer IM Medications, as needed, as dictated by the OMH Restructuring program
  • Any necessary projects or assistance to meet the continuing needs of the patients


Graduate of an accredited school for Licensed Practical Nursing; Minimum of two (2) years of clinical experience, preferably in an ambulatory care setting; Bachelor’s degree in Social Work, Psychology or related field preferred;

Current Basic Life Support (BLS) Certification required; Must be flexible, open to change and willing to participate in new initiatives in the department. EMR experience required, knowledge of E-Clinical Works is preferred
