Perinatal Case Manager: position is responsible for general supervision, provides diagnostic interviewing for all prenatal patients, counsels those individuals who would benefit from time limited, symptom focused psychotherapy or social support, triage those individuals who have needs that cannot be addressed here, refer patients to specialty programs, communicate with outside agencies regarding referral and follow-up of patients, provide crisis support and triage to all women's health patients and patients across the organization as deemed appropriate.
Requires strong interpersonal communication skills and knowledge of community resources for a range of medical and social services, as well as, skills related to both diagnostic interviewing and psychotherapy.
Eligibility requirements: Masters Degree in Social Work, Counseling or related field and fluency in English and Spanish with the ability to speak in a professional and courteous manner is required. Must possess strong customer services skills, a positive attitude and the ability to be proactive, resourceful and flexible. Basic computer skills with windows-based programs and experience with electronic health records is also required.
Ability to work one or more Saturdays per month and a few late nights per week or as needed.
Desired Skills: Experience working with prenatal patients in a primary care setting preferred.
Selection Criteria: Understands state regulations; computer literate; detail oriented; highly developed interpersonal skills.
