BROOKLYN PLAZA MEDICAL CENTER, INC., a Federally Qualified Health Center located in Fort Greene, Brooklyn, has immediate openings for a number of Full-Time (40 hours a week) and one (1) Part-Time Substance Abuse Counselors (CASAC).
RESPONSIBILITIES: Reporting to the Social Services Clinical Coordinator, all Substance Abuse Counselors will work as part of an interdisciplinary team providing psychosocial support as well as substance abuse treatment to patients as well as the appropriate support for patients’ family members and partners.
This position requires initiative and the use of independent judgment to, amongst other responsibilities:
Manage the care of patients in collaboration with the practice team to emphasize a team- based approach to care for patients resulting in full integration of primary care and behavioral health including, but not limited to, substance abuse services.
Interview as well as screen patients, review records and communicate relevant information to other members of the interdisciplinary team verbally and through documentation on the patient’s electronic health record.
Counsel patients in individual sessions to assist them in coping with substance abuse problems.
Counsel and help family members and/or partners to aid patients in supporting, and understanding the patient.
Monitor, evaluate, and record patient progress with that of treatment goals.
Collaborate with nurses, physicians, and counselors as well as behavioral health personnel to coordinate and plan treatment.
Refer patients and/or family members to community resources/treatment facilities for therapeutic care.
Other Responsibilities: Participate in in-service training and continuing education programs that are appropriate to job responsibilities; Provide monthly statistics to the Social Work Department to account for time and activities performed; Perform other duties as required and/or assigned.
B.A. Degree and New York State C.A.S.A.C. with at least one year’s experience working within the substance abuse field providing treatment.
New York Certification
Experience working with underserved populations including those living with HIV/AIDS, MSM, LGBTQ, as well as those formerly incarcerated.
Thorough knowledge of principles, procedures, and methods for addiction from a bio-psycho-social perspective.
At least one year of experience in a community-based health care facility from a bio-psycho-social perspective.
Excellent interpersonal as well as oral and written communication skills.
Computer literate and experience with electronic health records.
Ability and willingness to work on a team.
