We are currently recruiting for a Registered Nurse – Phone Triage.
Under the direction of the Supervisor, Nursing, the RN functions as a member of the interdisciplinary health care team in providing the highest quality of care to the patient.
Coordinate Care for Trillium Clients
Work with the interdisciplinary health care team to facilitate care
Work with interdisciplinary health care team to keep patients engaged
Actively participate in quality improvement initiatives
Demonstrate positive communication and teamwork within clinic and between departments
Receive and respond to incoming patient phone calls (Primary Focus of this role)
Triage patient needs using the EHR and coordination with clinic nurses and providers
Assign priority rating and refer to providers as needed. When appropriate resolve patient problem by communicating care instructions and/or advice.
Facilitate completion of medical forms needed for patient care
As a supportive float RN (secondary, lessor focus of this role) assist providers and nursing team members in delivering patient care.
Explain procedures and treatments to patients to gain cooperation, understanding and allay apprehension
Aid providers during examinations and treatments, as requested
Observe patient and record significant reactions/conditions. Notify providers of reaction to drugs, treatments.
Confer with providers; assist with written prescriptions, refills, phone-ins to pharmacies, and documentation on medication list
Administer treatments, injections, immunizations
Patient education on self-injection, new medications, or other treatments or issues related to care
As needed, perform duties of Medical Office Assistant taking vitals and updating patient information, collecting point of care samples from patients such as urine. Perform tests such as EKG as required
Maintain supplies, including the stocking and ordering of patient specific supplies as needed. Dispose of expired or damaged supplies
Perform routine scheduled inventory of patient medications, sample drugs and injectable medications, O2, and testing of defibrillators
Flex as needed to cover nursing absences and participate in nursing On-Call rotation
Complete a minimum of 10 hours of continuing education annually
Practice within the NYS RN scope of practice and maintain annual competencies
Document all patient care clearly, thoroughly, accurately, and timely
Order and maintain inventory of medications/ supplies
Maintain ongoing list of patients’ preventative care needs
Administer employee health information. Administer immunizations and PPD tests as needed. Record results in employee health file. Establish and maintain schedule to ensure all employee information and tests/injections are updated and administered on a timely basis as required by Health Department regulations.
Participate in the quality assurance program monthly CQI/QA activities as requested
Participate in clinical team and case management activities
Participate in the training and clinic orientation of new staff

Must possess a valid NYS RN license. Previous experience working in a hospital or clinic setting required. Knowledge of HIV/AIDS extremely helpful. Excellent written/oral communication and computer skills are required.
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
